Geographical Information Services (GIS) is a powerful tool for creating a new vision for health improvement and allocation of health services in communities. It gives both public health and health systems the ability to create a collaborative approach to conducting community health needs assessments and provides opportunities for strategic partnerships to address socially complex neighborhoods and communities.

In this webinar, attendees will be exposed to the lessons learned from within a health system implementing a real-time community health management system to provide geographically enabled health utilization information to influence strategic resource, patient care, and population health decisions.
